- Cross-reactive proteins: some assays may pick up antibodies to the related protein IA-2β (phogrin) — this is usually desirable, since phogrin antibodies have similar clinical meaning.
- Negative does not rule out T1D: always pair with GAD-65 and ZnT8.
- Borderline values: repeat in 4 to 8 weeks before drawing conclusions.
- Assay variability: use the same lab when monitoring serial titers.
What Happens After a Positive Result
- Review the rest of the antibody panel (GAD-65, ZnT8, anti-insulin) and calculate the number of positives.
- Check c-peptide to gauge current beta-cell function.
- Confirm diabetes status with fasting glucose, A1C, and/or OGTT.
- If positive in someone not yet diabetic, refer to specialized clinic or TrialNet study for monitoring.
