What “Long Term” Means in GLP-1 Research
For most published trials, “long term” currently spans 1-3 years of continuous use. The longest randomized extension data come from STEP 5 (2 years on semaglutide), SUSTAIN 6 (2.1 years average), LEADER (3.8 years on liraglutide), and SELECT (mean 39.8 months on semaglutide). Registry and post-marketing surveillance fill some of the gap beyond trial windows but cannot substitute for randomized data. Long Term Weight Outcomes
STEP 5 Two-Year Data
STEP 5 randomized 304 adults with obesity (without diabetes) to weekly semaglutide 2.4 mg or placebo for 104 weeks. Mean weight loss was 15.2% on semaglutide vs. 2.6% on placebo. Improvements in waist circumference, blood pressure, and lipid markers were also sustained.
Tirzepatide Long Term Data
SURMOUNT-4 evaluated whether continued tirzepatide maintained weight loss after a 36-week lead-in. Participants who continued tirzepatide maintained or extended weight loss; those switched to placebo regained substantial weight. Total study duration was 88 weeks.
What Happens After Stopping
The STEP 1 extension and SURMOUNT-4 both confirm a consistent pattern: most weight loss is regained in the year after discontinuation. Modern obesity medicine increasingly frames GLP-1 therapy as a chronic treatment, similar to antihypertensives.
Long Term Cardiovascular Outcomes
SELECT (Semaglutide)
SELECT randomized 17,604 adults with overweight or obesity and established cardiovascular disease (no diabetes) to weekly semaglutide 2.4 mg or placebo. Over a mean follow-up of 39.8 months, semaglutide reduced major adverse cardiovascular events (cardiovascular death, nonfatal MI, nonfatal stroke) by 20%. Results were published in the New England Journal of Medicine in 2023.

Long Term Adverse Event Signals
Gastrointestinal
GI symptoms (nausea, vomiting, diarrhea, constipation) typically improve over the first months and remain low long term. Persistent nausea or vomiting beyond the titration phase warrants evaluation.
Pancreatitis and Gallbladder Disease
Both are listed FDA warnings. Long term and meta-analytic data show small but real increases in gallbladder events with weight loss generally and with GLP-1 therapy specifically. Pancreatitis signals remain under surveillance but have not consistently risen above background rates in large pharmacovigilance reviews.
Thyroid C-Cell Tumors
Boxed warnings reflect rodent data. Human registry surveillance to date has not confirmed an increased risk of medullary thyroid carcinoma in humans, but follow-up is ongoing.
Lean-Mass and Bone Loss
Rapid weight loss of any cause reduces lean mass. STEP and SURMOUNT subgroup analyses suggest roughly 25-40% of weight lost may be lean tissue without resistance training. Adequate protein intake (typically 1.0-1.6 g/kg) and resistance training are recommended to preserve muscle.
