Why BMI Matters for Metabolic Health
BMI correlates reasonably well with body fat at the population level. Higher BMI is associated with increased risk for type 2 diabetes, high blood pressure, heart disease, and certain cancers, according to the CDC. In particular, BMI of 25 or higher roughly doubles the risk of developing type 2 diabetes compared with normal weight, and risk climbs steeply above 30.

What BMI Cannot Tell You
BMI is a blunt instrument. It does not measure:
- Body composition: muscle weighs more than fat, so athletes often score “overweight.”
- Fat distribution: abdominal fat carries more cardiometabolic risk than hip fat, yet BMI treats all fat equally.
- Bone density or hydration status.
- Metabolic markers like blood sugar, cholesterol, and blood pressure.
- Frame size or muscular build.

Waist Circumference
Measure around the narrowest point of the waist, usually just above the belly button. According to NHLBI guidance, a waist circumference above 35 inches (89 cm) for women or 40 inches (102 cm) for men is associated with elevated metabolic risk, independent of BMI.
Waist-to-Height Ratio
Divide waist circumference by height. A ratio above 0.5 has been linked with higher risk of type 2 diabetes and cardiovascular disease in multiple meta-analyses.
Body Fat Percentage
Methods include DEXA, bioelectrical impedance, and skinfold calipers. Accuracy varies; DEXA is the gold standard but costly.
BMI Limitations by Group

That level of change is usually sustainable with a combination of dietary adjustments, consistent physical activity, and sleep.

How Often to Check
BMI changes slowly. Rechecking every few months is usually enough. Quick swings likely reflect hydration or measurement error rather than true body composition change. When BMI Is Not Enough
Some people with normal BMI have elevated blood sugar, elevated triglycerides, or fatty liver — sometimes called “metabolically obese normal weight.” Waist circumference, fasting glucose, A1C, and liver enzymes can uncover these hidden risks. Conversely, someone with a BMI of 28 but a waist size under the risk threshold and normal labs may not need to treat that number as a health problem.
Putting It All Together
Here is a reasonable approach for most adults:
- Calculate BMI once every few months.
- Measure waist circumference alongside it.
- Check A1C, blood pressure, and lipid panel at least yearly.
- Discuss the whole set of numbers with your clinician rather than focusing on BMI alone.
