Why GMI and A1C Sometimes Disagree
A consistent gap between the two numbers is not a defect; it is biology. Common reasons:
- Red cell turnover. If your cells live longer than average, they have more time to glycate, raising A1C for the same average glucose. Shorter-lived cells lower it.
- Hemoglobin variants. Sickle cell trait, HbC, HbE, and other variants can interfere with specific A1C assay methods.

Limitations
- GMI depends on adequate sensor wear — aim for at least 14 days with greater than 70 percent data capture.
- Short windows (7 days) are volatile; 30-day GMI smooths noise but reacts slower.
- GMI cannot correct for CGM sensor drift — if your sensor consistently reads high or low versus fingersticks, GMI will inherit that error.
- Some people have a persistent personal glycation gap that makes their lab A1C always higher or lower than GMI. This is real and does not mean either number is wrong.
